blank Nonresident Alien Reimbursement/Honoraria Paperwork Requirements

To comply with University policies, you will need the following paperwork for:

Travel Reimbursements:

  1. Copy of the non-resident alien's passport (usually the first 2 pages including the picture).
  2. Copy of the I-94 form (acquired when entering the United States) OR Visa (neither are necessary if the Nonresident Alien's country is in the "Visa Waiver Program").
  3. Completed "Compliance Form" (for B-1, WB OR B-2, WT Visas).
  4. Original itemized receipts.

Exceptions:
Canadian residents, Resident Aliens OR Permanent Residents only need original itemized receipts.

Honorarium Requests:

  1. Copy of the non-resident alien's passport (usually the first 2 pages including the picture).
  2. Copy of the I-94 form (acquired when entering the United States) OR Visa (neither are necessary if the Nonresident Alien's country is in the"Visa Waiver Program").
  3. Completed "Compliance Form" (for B-1, WB OR B-2, WT Visas).
  4. Completed Form 8233 (must have original signature).

    If this form isn't provided, the University will withhold 30%.

In order to avoid the 30% withholding, the Nonresident Alien must have an ITIN (Individual Taxpayer Identification Number) and write that number in box 2 of Part I.

Exceptions:
Canadian residents, Resident Aliens OR Permanent Residents do not need any additional paperwork.

Submit the necessary forms along with a request, on a separate piece of paper, to the Accounting Office. The request must include the following information:

  1. First and last name
  2. SS #
  3. Home address
  4. Org/Fund (chartfield) to charge
  5. Approval signature
